Choices in Senior CareOwner and founder, Mary Wegman, is a highly experienced RN CCM (Registered Nurse Certified Case Manager) and GCM (Geriatric Care Manager). She has practiced in the East Tennessee community for over 20 years. Her extensive experience includes case management for the geriatric and disabled population, as well as in depth interaction with many of the areas home health, non-medical home care, and hospice providers. Mary is a member of the Case Managers Society of America and the National Association of Professional Geriatric Care Managers. Choices in Senior Care hires only those case managers who are registered nurses with much experienced across the continuum of health care, including home based case, hospital care, rehabilitation facilities, and residential facilities.

Geriatric Care Management Services:

  • Advocacy for the client and family, providing a strong voice for you.
  • Assessment of client by experienced Registered Nurses, pinpointing your most urgent needs.
  • Assisting the long distance caregiver with professional care management when you can't be there.
  • Guiding client through identification of goals and development of an individualized care plan, saving you money and time.
  • Implementation of care plan by screening, arranging and coordinating services, whether that be home services or facility placement, choosing the very best providers from the start.
  • Education on senior care issues, health and disease management and cognitive stimulation, teaching you to be proactive in your health care.
  • Coordination of care between client, family, and all professionals, preventing confusion and duplication of services.
  • Evaluation of services rendered, assuring that you are getting that for which you have paid.
  • Management of after-hours crisis situations, providing assistance when you can’t wait.
